During the household assessment, the team identified a one year old child with visible signs of severe malnutrition (MUAC red zone) and recorded missed immunisation follow ups. The case was documented and the caregiver was advised and supported with an urgent referral to a nearby health centre. This action enabled immediate access to medical review and set a basis for continued nutrition and health follow up.
